Thank you for your interest in our level one “Don't Be a Victim” women’s self-defense seminar. We are pleased to extend to you our group rate for this event. Please read the terms and conditions below, and fill out the group rate authorization form.

1. All members of your party must be present at the event in order for the group rate to apply.
2. Admission fee must be paid in full before the start of the event. 
3. The group rate event coordinatior should have sent you an group rate authorition code (GRAC), if you did not receive on, please call or text Charles Gooden at (808) 379-5084, or e-mail him at vma.vmahi@gmail.com 
4. If all members of your party are not present at the event, group rate prices are null and void, and revert to regular event admission prices.
5. Because this a certification seminar, please list each member of your group below. Substitutions are OK; however, please alerts a staff member of change(s) so proper completion certificates may be issued.   
6. Contact us for a new group rate authorization code if your headcount increases and you qualify for a different group rate consideration.
